Claim Form Login

If you are/were a patient at UTMC residing in the United States who had a patient portal account with UTMC between January 1, 2015, to and through September 30, 2023., then you may be entitled to Settlement Benefits.

If you are a Settlement Class Member and wish to receive a payment, your completed Claim Form must be postmarked or submitted online on or before December 9, 2025.

Privacy Shield Pro Product: Each Settlement Class Member will automatically receive a code to enroll in the Privacy Shield Pro product.The code is listed at the top of the Notice you may have received.

Cash Payment: Each Settlement Class Member who files a timely, valid claim will have the option to select a cash payment of $25.00.

To start your online claim, you must login with your Class Member Login ID and Password. You can find your ID and Password on the Notice that was sent to you. If you are unable to locate your ID & PIN, please contact the Settlement Administrator by emailing info@UTMCSettlement.com or calling (833) 417-4897. Please request your ID & Password by providing your full name and mailing address.
